This is a bona fide model release form for a bukkake video. If you don't know what a model release form is, it's a legal document that grants the makers of a video the rights to sell your image. If you don't know what bukkake is, well, look it up.
There are a lot of amazing things going on in this document (i.e. "proof of pop"), but clearly, the best is that they actually spelled "bukkake" wrong (my intensive research has informed that it is correctly spelled with two "k"s, not one.). I don't even really think there's any need to comment beyond that...

Now, I've seen my fair share of porn, but, the fact that they tossed in "after the first 50" using the shoulder-shrugging nonchalance of a parenthesis is pretty impressive. Not only does this mean that there are 50 guys doing their milky business all over some chick's face, it also means there are countless other guys trying to blow their wads before 49 other guys do just so they get paid that day.
